Performance Evaluation Forms

The Personnel Department provides Performance Evaluation forms (also known as "PPARs" 1) for bargaining unit employees (filename: pparbu.x) and non-bargaining unit employees (filename: pparnbu.x) in five formats:

MS Word 6
Macintosh users go to Public Disk 1, Templates and Forms folder
Windows users go to W: Templates-and-Forms/Word or rtf
Rich Text Format (RTF)
Macintosh users go to AppleTalk Zone: Mac Servers, Public Disk 1, Templates and Forms folder
Windows users go to W: Templates-and-Forms/Word or rtf
Adobe FrameMaker 5
(Unix users, Floras only, used comand maker5)
Adobe FrameMaker MIF
Macintosh users go to AppleTalk Zone: Mac Servers, Public Disk 1, Templates and Forms folder
Unix users go to /afs/slac/public/doc/forms/ppars98
Windows users go to W: Templates-and-Forms/Frame
Text Format

These forms are also available via the WWW at ftp://ftp.slac.stanford.edu/doc/forms/ or via anonymous FTP to ftp://ftp.slac.stanford.edu/doc/forms/ppars98/. If your browser is configured properly, you can click on these links to open the files through in the application through the browser. 2 Alternatively, you may "click and hold" (Mac) or click your right mouse button (Win or Unix) on the page to choose the "Save this link as..." option to save the file to your local directory or disk.

1. PPAR includes all Performance Evaluation forms and is a reference name for those forms.

2. Your browser must be configured to recognize the appropriate file extensions (.doc, .rtf, .mif, .fm, etc.), otherwise, you will merely see symbols and squiggles in your browser window. In Netscape, adjust your preferences in Navigator:Applications (4.x) or Helpers (3.x).
