Please attach a digital image of a government-issued ID of the person being impersonated: Your ID or the ID of the person you are authorized to represent
We will permanently delete our record of this digital image from our servers once we have used it to authenticate your report. The identification you provide: - Must be government-issued (ex: passport, driver's license) - Must be in color - Must clearly show the full name, date of birth, and photo If possible, please save this file as a JPEG.
