Collaborative Health Outcomes Information Registry (CHOIR)

(The Stanford-NIH Pain Registry is now called CHOIR)

In response to the Institute of Medicine Report Relieving Pain in America, and in partnership with the National Institutes of Health, we have developed and implemented an open source, open standard, and free National Collaborative Health Outcomes Information Registry (CHOIR) system.

We currently have the following partners, who together are contributing to building the infrastructure and expanding its use in clinical practice and in biomedical research.
