Stanford Clinical Hematology Rotation
The clinical hematology rotation provides three months of intensive training in the basics of adult and pediatric hematologic disorders, both neoplastic and benign. The rotation is designed to develop proficiency in automated cell counting/complete blood counts, peripheral blood smears, CSF and body fluids, HPLC/hemoglobin electrophoresis, urinalysis, and bone marrow interpretation, including correlating morphology with ancillary diagnostic tests, in particular, flow cytometry, immunohistochemistry, cytogenetics, and molecular diagnostics. These skills are attained through resident experience in initial case review, teaching-based case sign-out, report writing, and the use of an extensive collection of teaching slide sets. Residents also spend time in each area of the hematology laboratory including specimen processing, automated hematology, urinalysis, RBC special studies and special hematology.
The rotation is evenly divided between the Clinical Laboratories at Stanford University Hospital and Hillview. While at the hospital, the resident is responsible for the review and sign-out of peripheral blood smears, CSF/body fluid morphology, and urinalysis/body fluid crystals. In addition, the resident reviews cytogenetic and molecular genetic results and integrates the findings into hematopathology reports. While at Hillview, the resident provides an initial evaluation of specimens sent for flow cytometry, selects the appropriate panel of antibodies, interprets the flow cytometry data, signs-out cases, and writes up reports. Residents on the hematopathology rotation are an integral part of the clinical response for patients with newly diagnosed leukemias, providing primary evaluation of peripheral smears (and/or body fluids), facilitating timely flow cytometry if warranted, and efficiently communicating preliminary results to the hematology team.
