The Community Child Care Council of Santa Clara County, Inc. is pleased to present the 2014 Annual Report of its Early Head Start Program. Early Head Start is a federally funded child and family development program for low-income children birth to age three and their families, as well as pregnant women. 4C Council administers the Early Head Start program for children and families in Santa Clara County providing comprehensive services to support the mental, social, and emotional development of children from birth to age 3. Dear Parent or Provider:

In order to process provider payment more efficiently, the 4C Council will start using a basic attendance sheet (copy enclosed) effective May 1st, 2015.

Completed attendance sheets must still be submitted to the 4C Council by the 5th calendar day of each month. Checks will be mailed to providers on the 15th calendar day.

You will now be able to download the attendance sheets from our website at 4c.org
We encourage you to make a copy of the original attendance sheet before submitting it for payment just in case the attendance sheet gets lost in the mail.

Estimado Padre de Familia o Proveedor de cuidado infantil:

Con el fin de procesar el pago de los proveedores de manera más eficiente, el 4C Council comenzara a usar una hoja de asistencia básica a partir del 1 de Mayo del 2015. (ver copia incluida)

Las hojas de asistencia originales correctamente llenas todavía deben ser entregadas al 4C Council para el día 5 de cada mes. Los cheques serán enviados por correo a los proveedores el día 15 de cada mes.

Le recomendamos que haga una copia de la hoja de asistencia original antes de entregarla al 4C Council.

También podrá imprimir las hojas de asistencia de nuestra página de web en 4c.org.
