Important instructions for submitting this form:

- This form should be used by J1 and J-2 Exchange Visitors who do not have access to Axess, for reporting address, email and phone updates when they move to a new U.S. residence address during their J program. All changes should be reported within 10 days.

- This form should only be submitted AFTER the initial 'Arrival Confirmation' form has been submitted.

NOTE: If you are a Postdoc, Visiting Student Researcher, or a Knight Fellow, DO NOT USE THIS FORM. Instead, you should make your address updates in the SEVIS (US) address fields in Axess according to these instructions.
