KAISER PERMANENTE
On-campus Kaiser Mobile Health Vehicle (MHV)
The Kaiser MHV will be providing FREE vaccinations by appointment only to Kaiser Health Plan Members at its two on-campus locations throughout the flu season. Flu vaccinations are also available to Kaiser members at no charge at each facility.
CAMPUS LOCATIONS
Maples Pavilion Parking lot, Campus Drive and Bonair Siding
Dates: Every Tuesday
Time: 8:30 AM – 3:00 PM
SLAC Security Office visitor parking lot, 2575 Sand Hill Road, Building 235
Dates: 1st Thursday of each month (October 1st, November 5th, December 3rd)
Time: 8:30 AM – 3:00 PM

- Select either the Maples Parking lot or SLAC locations from the drop-down menu
- Enter your personal details and Kaiser medical record number
