Transfusion Reaction Workup
It is required to follow a strict procedure whenever a transfusion reaction is recognized
- STOP THE TRANSFUSION
- Keep intravenous line open with physiologic saline
- Notify the patient's physician and the transfusion service (650-723-6445)
- Perform a bedside clerical check
- Return the unit to the blood bank
- Collect a specimen from the patient for Coombs test or DAT
- Document the reaction by filling out the report
