How to Use
The choice to declare your pregnancy is voluntary. If you choose to declare your pregnancy, do so in writing and a lower radiation dose limit will apply to your embryo/fetus. Please complete and fax this form to Health Physics at (650) 723-0632.
If you choose not to declare your pregnancy, you and your embryo/fetus will be exposed to the same radiation dose limits as other occupational workers.
