Privacy Incidents

Privacy Incident Definition:

A privacy incident is a suspected or confirmed adverse event or action that results in use or disclosure of personally identifiable information (PII) that results in non-compliance with privacy laws, regulations or policies and procedures.  

If you believe a privacy incident has occured, you must notify the Privacy Office immediately.  Even if it seems minor or inconsequential, report it to the Privacy Office so we can make the determination.  Immediate reporting to the Privacy Office is critical because, depending on the circumstances, federal or state laws may require us to take very specific remedial actions within defined timeframes.

Hospital Privacy Incidents
