Expert trainings, free resources on care transitions

Long-time Moore Foundation grantee, Eric A. Coleman, MD, PhD, recently enhanced his Care Transitions Program website, highlighting the new Advanced Care Transitions Intervention® training.

Professor of Medicine and Head of the Division of Health Care Policy and Research at the University of Colorado Anschutz Medical Campus, Eric is a national expert on care transitions. He and his team focus on training health care providers to improve quality and safety during the transition of patients from the hospital to their homes or other settings. Eric’s signature offering is a low-cost, low-intensity evidence-based intervention comprised of a home visit and three phone calls, called The Care Transitions Intervention® or also known as CTI® or the Coleman Model®. During a four-week program, patients with complex care needs and their family caregivers receive tools, work with a Transitions Coach® and learn self-management skills that will ensure their needs are met during the transition from hospital to home. The Advanced CTI training, aimed at Transition Coaches who have already been trained in CTI, focuses on training coaches to engage with family caregivers to further improve the transition experience for patients and families.

Trainings for organizations are designed to meet the needs of both large hospital systems to small clinics. Participants gain expertise in Care Transitions Intervention® content through web-based training portals and intensive full- or half day, case-based on-site trainings.
