The Community Partnership Program
Stanford Health Care dedicates considerable resources to improve the health and well-being of the community through its Community Partnership Program. The program is a multi-year, strategic investment in community health and consists of three major health initiatives:
- Improve the health and well-being of older adults
- Improve access to care
- Reduce cancer-related health disparities

Community health needs assessment (CHNA)
This assessment documents the process undertaken by Stanford Health Care to determine the health needs of the community it serves. The implementation strategy outlines how those health needs will be addressed.
Community programs
Stanford Health Care maintains a strong commitment to public service and the community. It dedicates significant resources to support its community programs. Our community programs range from ensuring health care access to the elderly and indigent patients to being actively involved in a wide range of community service programs to improve the health status of the community as a whole.
