BeWell phone option

If it is difficult for you to come to the Stanford campus for your screening and advising appointment(s), and you’ve seen your physician since November 1, 2015, then BeWell offers you a phone option.

After logging into the program and completing your SHALA, click “Screening” and choose “Option 2 – Screening by Physician.”

Confirming this choice will allow you to print a form and send your screening results to BeWell. Please note: October 15, 2015 is the last day to fax in health screening results from your doctor.

Once BeWell receives your screening information, you will receive an email reminder to sign up for an advising appointment by phone. Phone appointments are available most days of the week during both morning and afternoon hours.
