During the week before your admission, a representative from Patient Access Services may contact you. The purpose of this call will be to obtain:

• Information about insurance benefits and/or payment arrangements
• Current address
• Phone number
• Employer information (if any)

Depending on your insurance carrier, the representative may be able to answer your questions about co-payments or deductibles.

If you do not have insurance coverage, Patient Access Services will refer you to a financial counselor. Contact financial counseling before your visit.

You will need to have the following information with you when you check in:

• Insurance card
• A form of payment for any co-pays or deductibles (credit card or checkbook)
• Photo ID--any one of the following is acceptable:
  • Valid state-issued driver’s license
  • Valid state-issued ID card
  • Valid passport
  • Valid U.S. Military ID card
  • Valid U.S. Permanent Resident Card
• Advance health care directive, if you have one
• List of prescriptions (you do not need the actual prescriptions unless requested by your doctor). Or fill out our medication list (PDF).
  

In addition, bring any medical devices or appliances you may need, including eyeglasses, hearing aids, dentures, canes, etc.

You may wish to bring some or all of the following:

• Non-skid slippers and robe
• Comb and brush
• Toothbrush and toothpaste
• Toiletries
• Reading and writing materials
• Laptop/MP3 player
• A small amount of cash (please do not bring more than $30)

Small electrical appliances such as shavers and hair dryers are also acceptable, except in the Intensive Care Unit. This list is not all-inclusive. Different units have different rules about what they allow. If you have questions, ask your doctor's office. Someone there should be able to answer your question or direct you to someone who can.

If you have questions about the procedure itself, how to prepare for it, or what to do afterwards, call your doctor's office. If you are being admitted, you should receive a call from your doctor's office the night before that will help you with some of these questions, including where you need to go.
