Patient rights
Your participation in Stanford Coordinated Care is voluntary. If, at any time, you decide that the service does not meet your needs or that SCC is not a good fit, you may disenroll from receiving further services.
Your specific demographic and medical information will NOT be shared with anyone other than the Coordinated Care team and providers directly involved in the care of your health without your written consent or unless compelled by law. Anonymous and combined information will be used for clinic evaluation and quality improvement purposes. Results from evaluations will be made into reports that will be shared within the clinical community and other necessary parties to help improve the effectiveness of health care services.
Your access to the Stanford Coordinated Care team is available to you by phone, in-person communications, and e-mail via MyHealth. E-mail should be used for non-urgent questions and should not be used in emergencies or when you are in need of immediate response. All communications may be documented in Electronic Health Records (EHR).
Patient responsibilities
As part of delivering to you the most complete level of care, you allow Stanford Coordinated Care to receive information about your health care from current and/or previous physician care via our Electronic Medical Record (EHR) system, EPIC, through its “Care Everywhere” module.
If you have received medical care from a medical organization that does not participate in “Care Everywhere,” we need you to sign a release of information to your previous provider(s) to obtain your medical records before your first visit. Our physicians prepare for their first meeting with you by thoroughly reading your chart in order to give you excellent care from the start.
Because our SCC physicians are available after regular clinic hours, via our answering service, to enrolled SCC patients, we ask that our patients:
- Call SCC first before going to the emergency room, unless you cannot safely wait a few minutes to speak to us. Your SCC physician can provide reassurance, and can call ahead to the emergency department to let them know you’re coming as well as the reason.
- In major emergencies, call 9-1-1 rather than trying to drive to your nearest emergency department. Please notify us, or ask someone to call us at (650) 724-1800 within the first 24-48 hours of your visit to a hospital especially if you are admitted, so we can support you.
- We will do our best to provide our patients with a same-day, or next-day, appointment when an acute or urgent medical need arises.
If, at any time, either party feels that the other is not being supportive regarding the above statements, we promise to resolve the issue with you.
It is your responsibility, because Stanford provides our clinic and its services as a benefit for its active/current employees and their covered adult dependents, to notify us if your job has or will end for any reason, including retirement.
If your Stanford-based insurance benefits end, so does your ability to be a patient at SCC (unless your spouse or registered domestic partner also is employed by Stanford and adds you to their insurance coverage). Being a patient of Stanford Coordinated Care is reserved only for current/active employees and their adult dependents. *
