Go Pass on Clipper Lost, Stolen, Replacement form

Eligible employees, graduate students and postdoctoral scholars: Use this form to request a replacement Caltrain Go Pass and/or VTA SmartPass.

Activation and Reactivation

Go Passes for eligible commuters are valid for the full calendar year (through December). The Go Pass will be activated in four-month increments to avoid Clipper’s 180-day deactivation policy. If you remain eligible, Stanford Transportation will automatically renew your Go Pass every four months (i.e., April, August, and December), and you will not need to take any action.

*Hospital employees: not your badge number – your employee or file ID number is on your timecard or pay statement. If you are a medical resident or fellow and are having trouble signing in, please try using your six-digit hospital employee ID number (which is different from your SID). If you only have a five-digit ID number, please try adding a “0” at the beginning of the five digits.
