About Aging Adult Services

Thanks to medical advances and overall improved health, adults are enjoying independence, productivity, and vitality well into their 70s, 80s, and beyond. Aging Adult Services works closely with Stanford Senior Care to help you and your family with the resources, tools, and support you need.

Continuum of care

Aging Adult Services offers a variety of support services and resources for aging adults for all aspects of care including:

  • Inpatient services
  • Home care programs
  • Community support
  • Technological resources
  • Caregiver tools
  • End of life support

  • Transitional care: Assistance with transitional care from hospital to home, including education, consultations, and links to community resources
  • Dementia support program: Consultations and support for those coping with dementia
  • Heart support program: Inpatient, outpatient, and skilled nursing facility support

Community programs

  • Caregiver support program: One-on-one, short-term telephone support for family caregivers
  • Chronic disease self-management program: Community-based program offering self-management tools and techniques
  • Strong for Life: Muscle strengthening exercise program offered at several local senior centers
