Hospital Discharge Planning and Placement: What are the Options?

Anytime someone is discharged from the hospital there are many choices and decisions to make. Learn how to become an empowered patient advocate to help navigate the hospital discharge planning process. Being proactive in your decision-making and eliminating uncertainties will help provide for a calmer, more organized move.

In this seminar, we will discuss:
•  The roles of hospital staff
•  Policies regarding patient rights to appeal the discharge date
•  Post-discharge: skilled care vs custodial care needs
•  Insurance coverage: health care vs long-term care
•  Medicare coverage: skilled nursing and hospice care
•  Admissions process for Assisted Care facilities and Board and Care facilities

Discharge planning checklists and resources will be provided.
