Palliative Care for Cancer
The Stanford Palliative Medicine Clinic at the Cancer Center provides treatment and counseling for physical and emotional needs during cancer treatment. Examples include pain, fatigue, nausea, depression, anxiety, and spiritual issues. Additionally, the team can assist with documentation of an advanced directive and goals of care. Treatments offered integrate medical, psychological, social work, and spiritual approaches. Care is coordinated with cancer treatments.

About the Palliative Medicine Clinic at the Cancer Center
Welcome to Stanford Palliative Care, a dynamic and innovative team dedicated to providing you the best possible quality of life when confronted with a serious illness. Our team includes board certified doctors in palliative medicine, advance practice nurses, and social workers. We work with you and your family to address symptoms, such as pain, and provide assistance with difficult and complex treatment choices.
Our team approach allows us to address your physical, social, psychological and spiritual needs. We work with your doctor to provide you compassionate, coordinated care that we tailor to your and your family's life goals.

Please fax the Medical Record Release Form to your new patient coordinator. The medical release form is an authorization form for external facilities to release medical records to Stanford Health Care (formerly Stanford Hospital & Clinics).
