If you need to reschedule your appointment, please call us at least one week prior to your visit.
PREPARE FOR YOUR APPOINTMENT
- Complete and return the food frequency questionnaire 7-10 days before your visit.
- Our dietitian will have a full analysis ready at your visit.
- Mail or bring any medical or personal records related to previous hospitalizations, cardiovascular risk factor values and/or treatment.
- Bring your completed cardiovascular clinic patient questionnaire.
- Bring results from prior lab tests, if requested.
- Plan on being at the clinic for approximately one to two hours.
