The Santa Clara County Public Health Department, Vital Records and Registration is responsible for registering all of the births and deaths that take place within the county of Santa Clara only.
Birth and death certificates, dating back from 1873 to the present, are available at the Santa Clara County Office of the Clerk-Recorder or call 1(408) 299-5688.
Out-of-Hospital Birth Registration
- Out of Hospital Birth Packet
- Any hospital documentation as proof for mother and newborn
- (Example: discharge papers, letter from birth recorder acknowledging the birth, etc.)
- Please also advise parent(s) that it is important that they contact us as soon as possible, to schedule an appointment to complete the registration for their newborn’s birth in order to avoid delays.
Correcting or Amending Certificates
The California Department of Public Health - Vital Records has published a variety of informative pamphlets that describe different amendment options in detail—from correcting a minor spelling error to adding a father to the birth certificate. Not sure which form you need? Original forms must be used and submitted to the state. Forms can be obtained from our office, a local registrar office in another county, any County Recorder’s office, or ordered online from the
California Department of Public Health - Vital Records Forms page, or call us at (408) 885-2010 for more information.
