Only use this form if you are submitting a Late Application to Graduate. Students who applied to graduate on time must change their diploma distribution through Axess.

• Note: For Spring Quarter only, students who apply for Spring degree conferral can change their diploma distribution option in Axess up until May 31.

Late Application to Graduate for Bachelor's Degree 
Apply to graduate late through Axess until the late application for degree conferral deadline. There is a $50 fee for applying to graduate after the deadline.

Late Application to Graduate for Advanced Degree
Apply to graduate late through Axess until the late application for degree conferral deadline. There is a $50 fee for applying to graduate after the deadline.

Walkthrough Form
Use this form to petition your department or school to allow you to walkthrough a Spring Commencement Ceremony without graduating Spring Quarter.

Withdrawal of Application to Graduate 
Use this form to withdraw your application to graduate until the late application for degree conferral deadline.
