Protocol Statement of Support (PSOS) for Initial Application

Click on the PSOS link within the appropriate CRG category in the table above. Download and complete the entire PSOS form and submit the form to the SRC. If your study does not fall under one of these groups please use the various/other category above or contact the SRC office at (650) 724-4926.

Cancer CRG Eligibility Flowcharts (FC) (for initial application only)

For all interventional trials, click on the FC link within the appropriate CRG category in the table above. Download the appropriate CRG flowchart. Indicate where this protocol fits in relation to other studies currently open or planned (see flowchart instructions). If the appropriate flowchart does not exist, create a new flowchart using the blank protocol template.  Mark up a copy by hand and submit it to the SRC. For questions or assistance with flowcharts contact Sarah Pelta.

Protocol Statement of Support for Renewal (RPSOS) Application

Click on the RPSOS link within the appropriate CRG category in the table above. Download and complete the entire RPSOS form and submit the form to the SRC.

How to Submit a General PSOS (Various/Other) for Signature

Dr. Mark Pegram, Associate Director of Clinical Research, will sign the PSOS for studies that do not fit into a specific Clinical Research Group. Please complete the form except for #4 and the signature on the last page. Email the completed form along with a copy of the protocol to Dr. Pegram. Then follow up by scheduling a short meeting between the PI of the study and Dr. Pegram by contacting Dr. Pegram's assistant, Carol Fernandez at carolf2@stanford.edu, (650) 723-5801. As a backup please contact Dr. Susan Knox, Director of the Protocol Review and Monitoring System, to schedule a meeting for her signature.
