We offer on-site training for the Chronic Disease Self-Management Program at Stanford, and web-based cross-training for Diabetes Self-Management, Chronic Pain Self-Management, Cancer: Thriving and Surviving, Positive Self-Management (HIV), Tomando Control de su Salud (Spanish CDSMP), and Programa de Manejo Personal de la Diabetes. At this time, we do not offer regular training for the Arthritis Self-Managment Program or the Curso de Manejo Personal de la Artritis (Spanish Arthritis Self-Management Program). If you are interested in the English or Spanish ASMP, please contact us. Those interested in programs other than the core program, CDSMP, must take a full on-site CDSMP training before they may take a cross-training for other programs.

Trainings for representatives of health care organizations are 4½ days. It is strongly suggested that health professionals bring a lay person with chronic disease with them. All workshops and trainings are designed to be facilitated by 2 people, so it is important that any new organization send 2 people to the training, or they will be unable to implement the program.

Each trainee receives a detailed Leader's Manual, a copy of the workshop's textbook and audio CDs (if applicable). Those being trained as Master Trainers (trainers of Leaders) will also receive a Master Trainer's Manual.

Cross-Training
Because the process of and many parts of our programs are based on the Chronic Disease Self-Management Program, we offer cross-training programs for our other programs. Cross-trainings are held online in a web-based environment. Trainees interested in any of our programs should take the CDSMP Master Training first. We offer cross trainings for anyone wishing to offer the Diabetes Self-Management Program, the Positive Self-Management Program (HIV), the Chronic Pain Self-Management Program, the Cancer: Thriving and Surviving Program, the Tomando Control de su Salud program (Spanish CDSMP), or the Spanish Diabetes Program (Programa de Manejo Personal de la Diabetes) to those already trained as Master Trainers for the Chronic Disease Self-Management Program or Tomando Control de su Salud.

Though we do not give a specific training for the Spanish or Japanese versions of the PSMP, bilingual trainees may attend the English cross-training and purchase a license for the Spanish or Japanese versions. The Spanish and Japanese versions are translations that are so similar to the English PSMP that additional training is not required.

Away Training
We can also come to you. Two Stanford trainers will train up to 24 Leaders and/or Master Trainers at your facility. Master Trainers will enable you to sustain the program in your organization, as they can serve as your own on-site trainers for the future. You can combine your training with other organizations in your area to reduce your costs, if you wish. Demand for these trainings is great, so we need to plan 4-6 months in advance. Requirements of Training

Hours of the CDSMP on-site training are 9:00 AM to 4:30 PM, days one through four, and 9:00 AM to 12:30 PM day five.

You must attend ALL of the training in order to be licensed to use the program and authorized to train leaders. Make any travel or commuting arrangements with that in mind.

Do NOT make return flight arrangements to depart before 4:30 PM on on the last day of the training.

Accommodations

Click here for a list of nearby hotels and motels. We suggest you reserve your room early, regardless of where you stay. Breakfast will be served at 8:30 am at the training site each day, and lunch will be served the first four days.

Transportation

Palo Alto and Stanford are near both the San Francisco and San Jose International Airports. Most people find the San Jose Airport slightly more convenient. Airport shuttles that bring you to Stanford usually require a reservation. If you arrive with a car, there is a $12 per day charge for parking (except Saturday), which is due with registration and is non-refundable. As with most college campuses, parking is not easy, but can be found within a block or two. Visitors with a handicapped parking placard may park in any legal parking space anywhere on campus at no charge. Reserve a parking permit on your application.

If you do not arrive with a car, or don't wish to search for parking each day, Stanford runs a free campus shuttle from 6:00 AM to 8:00 PM, Monday through Friday (none on Saturday). The "Marguerite" (named after the horse that pulled the shuttle wagon in the early days) also runs to the transportation hub on University Avenue in Palo Alto, where you can catch the train or the bus to other destinations.

The Palo Alto/Stanford area is very easy to get around on a bicycle. You can rent a bicycle from the Campus Bike Shop.

Special Services

Participants or attendees needing special arrangements to accommodate a disability may request special services by indicate their requests on the registration form. Participants with dietary requests may also indicate so on the form. We will do our best!

Stanford University is a smoke-free campus. Smoking is prohibited.

Because the Programs are most powerful when taught by peers with chronic disease, we offer tuition at a substantially reduced rate for lay persons. We hope this will help make it possible for your facility to send at least one peer to be trained. Having people with chronic disease at the training also enriches the training. Since the "secret" of the success of the workshop is about process, not content, peers with chronic disease make the training more effective.

Registration

To register for training, fill out the online registration form, and pay by credit card , check, wire transfer or PO by the deadline. Call or email Gloria Samuel at (650) 723-7935 self-manage-training@stanford.edu to confirm space availability. Enrollment is strictly limited to 24 per training, on a first come, first served basis, and a minimum enrollment number must be reached before a training is confirmed. In order to assure a place in training, submit your application and payment for your tuition as soon as possible, as the trainings sometimes fill months beforehand. The first 24 complete applications with payment in full for a specific training we receive will be guaranteed a spot if the minimum enrollment number is reached. You will receive a training confirmation email when your payment is received and spot is confirmed so that you can make travel arrangements. Do NOT make travel arrangements until you receive your confirmation!  Your licensing agreement will be sent by email prior to training date, and routed to your facility's program license contact. Tuition is nonrefundable after the application deadline. We strongly encourage agencies to send at least two trainees, one or both a lay person with chronic disease, since all leader training is facilitated by two trainers. You will not be able to implement your program without at least 2 people trained. Please fill out an individual application per person for your agency together. There will be no refunds after the application deadline!

Palo Alto/Stanford Area Facilities

There is a lot to see in the Palo Alto area. On the Stanford University Campus, the Memorial Church, Hoover Institution, Rodin Sculpture Garden, Papua New Guinea Sculpture Garden and the art gallery are popular sights. If you decide to combine your trip to our training with a vacation, there is no end to things to see and do in and near the San Francisco Bay area. A short trip will find you in San Francisco, San Jose, the redwoods or the coast. Monterey and Carmel-By-The-Sea are also wonderful day trip destinations. You can shop 'til you drop at the Stanford Shopping Center. Bloomingdale’s, Macy's, Nordstrom, and Nieman-Marcus are complemented by a myriad of smaller, upscale shops and restaurants in a beautiful, garden setting.
