Care Coordination Liaisons
Care Coordination Liaisons support individual patients and families through the continuum of healthcare ensuring that barriers to care are resolved and that each stage of care is accessible for the family. Care Coordination Liaisons will assess resource needs and provide appropriate referrals based upon an assessment of needs. Care Coordination Liaisons will collaborate with the Social Workers, Case Managers and Housing and Transportation Coordinators regarding resource assessments and referrals.
Care Coordination Liaisons assist patients and families in a wide variety of ways such as:
- Addressing barriers to care such as food assistance, housing and transportation assistance referrals.
- Public assistance referrals such as Paid Family Medical Leave, Social Security Disability, WIC
- Accessing resources in the community
- Insurance
- Discharge planning
