Licensing Information

State Medical Boards

Submit state medical licensure forms to the School of Medicine Registrar's Office by email to medregistrar@stanford.edu or by mail.

Please carefully review the instructions the state medical board licensing forms, as every state is different. In California, a copy of your diploma is required for the Registrar’s Office to certify with your licensure forms, and you will need to provide a PDF scan or copy of your diploma when submitting the licensure paperwork. The licensure forms will be completed within 1-2 weeks and a notification will be sent to you when they are completed.

Federation of State Medical Boards (FSMB)

For alumni using the Federation of State Medical Boards (FSMB) Federation Credentials Verification Service (FCVS), complete the FCVS application, including uploading a copy of your MD diploma and submitting an electronic transcript to FCVS.

Graduates from 2016 onward will have electronic diplomas and electronic transcripts on file at the FSMB if the graduate opted to send the electronic documents to FSMB when applying to graduate.
