Undergraduate Forms

Undergraduate petitions related to academic standing and policies are administered by the Office of the Vice Provost for Undergraduate Education (VPUE), Sweet Hall, 1st floor. For procedures and petitions, see the VPUE petitions page.

Included on the pages following are forms for Undergraduate Study and Programs, Transfer Credit, Enrollment, and Coterminal Students.

The petitions and procedures on the pages following are administered by the Office of the University Registrar. Students should drop off forms at the Student Services Center, Tresidder Union, Second Floor.

Coterminal Student Forms

Forms related to coterminal students with active undergraduate careers are handled by the Office of the Vice Provost for Undergraduate Education (VPUE), Sweet Hall, 1st floor. Forms related to coterminal students who have conferred their undergraduate degree and have an active graduate program are handled by the Student Services Center, 2nd Floor, Tresidder Union.
