Stanford Cancer Survivorship Program
We provide supportive care during treatment, but understand that the cancer experience doesn't end with completion of intensive therapy. Some cancer-related issues can continue for years after treatment. In 2012, we opened the Stanford Cancer Survivorship Program to help survivors' transition from diagnosis to wellness through a variety of services and clinical care from our multispecialty team of survivorship experts.

Cancer Survivorship Program
Cancer Survivorship is more than surviving cancer. It starts from the time of diagnosis, continues after treatment and includes individuals who experience cancer as well as family, friends and caregivers.
More people today are surviving cancer than ever before thanks to advances in detection, diagnosis, treatment and follow-up in cancer care.
One in twenty adults in the US is a cancer survivor—numbers that have quadrupled since the 1970s. The need for long-term follow up care has grown with the numbers to address the ongoing issues that cancer and treatment present.
Cancer Survivorship starts from the time of diagnosis through post-treatment and includes individuals who experience cancer as well as family, friends, and caregivers. Stanford's Cancer Survivorship Program helps people transition from cancer diagnosis to wellness by providing support services and evidence-based medical care, including:
- Innovative education and resources
- Comprehensive clinical care from a team of experts
- Research studies on quality of life and survivorship
Once treatment ends, we realize the cancer experience continues. The Cancer Survivorship Program includes clinic visits with nurse practitioners and physician assistants, whose expertise includes cancer care and the specific issues survivors may face years after treatment. They offer methods to maintain and promote health during the transition from cancer treatment to wellness and work in conjunction with oncologists and other team members to provide the best possible care after treatment.
Clinical Trials
Clinical trials are research studies that evaluate a new medical approach, device, drug, or other treatment. As a Stanford Health Care patient, you may have access to the latest, advanced clinical trials.
Open trials refer to studies currently accepting participants. Closed trials are not currently enrolling, but may open in the future.

MEDICAL RELEASE
Please fax the Medical Record Release Form to your new patient coordinator. The medical release form is an authorization form for external facilities to release medical records to Stanford Health Care (formerly Stanford Hospital & Clinics).
