Support to Navigate Challenging Health Conditions
Stanford Coordinated Care (SCC) is a primary care practice with a team of medical professionals who help individuals who have chronic or complex illnesses lead healthier lives and better navigate their health care experiences.
Being a patient of Stanford Coordinated Care (SCC) is an exclusive benefit for:
- Current employees, and their covered adult dependents, of one of the following Stanford organizations:
- Stanford University (SU), or Stanford Linear Accelerator Center (SLAC)
- Stanford Health Care (SHC), or Stanford Children's Hospital
- Individuals whose primary insurance is Stanford Health Care Advantage (SHC-Advantage), a Medicare HMO plan available to residents of Santa Clara, San Mateo and Alameda counties

Our Primary Care Doctors
Stanford Coordinated Care (SCC) is a Primary Care Clinic whose Specialty is the Treatment of Chronic or Complex Illness
Being a patient at Stanford Coordinated Care includes:
- $0 copay to see your SCC primary care physician or other care team provider* (*varies by your insurance plan);
- 24/7 telephone access to your primary care physician at SCC. Our doctors are on call for their patients during evenings and weekends;
- Same-day and next-day appointments for our enrolled patients with urgent/acute needs;
- In-office blood draws for your lab tests;
- Highly trained and experienced Medical Assistant Care Coordinators who will:
- be your “point person,” communicating your messages or concerns with your SCC physician and care team;
- identify and plan with you, in achievable steps, how you can address and attain your personal health goals;
- keep in touch with you between visits, by phone or MyHealth messages to provide support or keep you on track, at your request;
- help you navigate the health care system, including specialty care and tests and procedures
- Coordination of your medical care with your medical specialists and their staff members. This includes having a member of your Care Team accompany you to visits, as needed and schedules permitting
- Our pharmacist who can review your medications and is available to consult with you;
- Our skilled clinical nurse specialist (CNS) who can assist and support you over the phone, as well as in scheduled clinic visits;
- Our licensed clinical social worker (LCSW) with whom you can discuss your concerns about your health and other matters;
- Our licensed occupational therapist (OTR/L) specializing in Feldenkrais-based movement awareness to improve function and reduce pain;
- Our registered dietician (RD) who can discuss your nutrition and food plan options with you, tailored to your individual goal(s);
- Our Stanford Health Librarian is available to help you acquire articles and other sources pertaining to the health topic(s) of your choice; and
- Access to attending Stanford's Better Choices, Better Health (BCBH) program, which is a six-week series of classes offered several times a year to help SCC patients learn BCBH’s self-management program.

AFTER HOURS
Please call your SCC physician first before going directly to the emergency room, unless you feel that you cannot safely wait a few minutes to speak to us.
We will make every effort to offer you an appointment the next day for urgent problems if the problem can safely wait until morning.
In major emergencies, call 911 for an ambulance or proceed directly to the nearest emergency room.
