Potential Community Partners
The following organizations have indicated an interest in partnering with H4A to provide service learning internships for H4A fellows. Appearing on this list is not a guarantee that a fellow will be assigned to a potential Community Partner, nor an endorsement of the potential Community Partner by H4A. Potential Community Partners must take part in the matching process and be matched with a H4A fellow for a service learning internship unless alternate arrangements have been made with H4A.
Asian Pacific Partners for Empowerment, Advocacy and Leadership (APPEAL)
“Asian Pacific Partners for Empowerment, Advocacy and Leadership (APPEAL) is a national organization working towards social justice and a tobacco-free Asian American, Native Hawaiian and Pacific Islander (AA and NHPI) community. Our Mission is to champion social justice and achieve parity and empowerment for Asian Americans, Native Hawaiians and other Pacific Islanders by supporting and mobilizing community-led movements through advocacy and leadership development on critical public health issues."
Arbor Free Clinic
“The mission of Arbor Free Clinic, one of two free clinics comprising the Cardinal Free Clinics, is to provide culturally appropriate, high quality transitional medical care for an underserved patient population and to educate and empower a new generation of healthcare leaders to proactively address health disparities and improve access to care in their communities. The Arbor staff is comprised of dedicated volunteer physicians and students.”
The Boys and Girls Club of the Peninsula
“The Boys and Girls Club of the Peninsula’s mission is to help at-risk youth develop the academic and life skills they need to complete high school ready for college or career. BGCP works with parents, corporate partners and schools to serve 1,700 kindergarteners through twelfth graders in East Palo Alto, Redwood City and eastern Menlo Park, implementing social and academic interventions that most impact a child’s achievement in school and in life, in order to close the achievement gap.”
Ceres Community Project
"At The Ceres Community Project, our goal is to restore whole foods to their place as the foundation for health, and to build relationships among all members of our community to support happier and healthier lives. We teach youth about growing, preparing and eating healthy organic whole foods by having them serve as the gardeners and chefs. Working together with adult mentors these young people provide 85,000 free organic and nutrient-rich meals each year to low-income people dealing with serious illness and their families in Marin, Sonoma and Alameda counties."
City Slicker Farms

The IHC specializes in medical, dental, counseling, nutritional, community wellness and outreach, and health insurance services.”
India Community Center

The ICC also serves as an essential resource for the community by providing free or heavily subsidized services in critical areas such as health, legal aid, and senior support.”
InnVision Shelter Network
“InnVision Shelter Network provides housing, educational, and medical services to single adults and families who are homeless or at-risk of becoming homeless. InnVision operates at several locations throughout the bay area, including the Opportunity Center, which serves Palo Alto and the Midpeninsula area.”

Leland Scholars Program

Mobile Adolescent Health Services Program - Teen Van
“Teen Health Van is a community outreach program, sponsored by Lucile Packard Children’s Hospital, that provides comprehensive primary health care services for homeless and uninsured adolescents in Santa Clara, San Mateo, and San Francisco Counties. Teen Health Van partners with schools and other community agencies, and all services (including medications and labs) are free of charge for all patients.”
Lytton Gardens
“Lytton Gardens is a vibrant community of health care and Senior care professionals, staff, volunteers, and senior residents. Lytton Gardens is a multi-level care facility that provides independent retirement living, assisted care living, and a skilled nursing facility.”

Stanford Coordinated Care
“Stanford Coordinated Care is a team of medical professionals and care coordinators who help people with chronic illnesses lead a healthy life and smoothly navigate their health care experiences. Services include assistance in managing chronic illness, coordination of medical care between specialists, and in-clinic primary care in coordination with primary care providers.”
Stanford Geriatric Education Center

Stanford Health Improvement Program
"The Stanford Health Improvement Program (HIP) is a division of the Stanford Prevention Research Center. HIP's mission is to improve the health of the Stanford Community, as well as communities across the nation and around the world. HIP offers over 200 group fitness classes every quarter for university and hospital faculty, staff, retirees and their family members. HIP also offers approximately 50 health education classes each quarter. Health education classes are available in group, individualized, and online formats, and are open to the general public, as well as members of the Stanford community. In addition to our classes, HIP provides trainings and consultations to other organizations on the topic of worksite health promotion."
Stanford Palliative Medicine Clinic at the Cancer Center
"The Stanford Palliative Medicine Clinic at the Cancer Center provides treatment and counseling for physical and emotional needs during cancer treatment. Examples include pain, fatigue, nausea, depression, anxiety, and spiritual issues. Additionally, the team can assist with documentation of an advanced directive and goals of care. Treatments offered integrate medical, psychological, social work, and spiritual approaches. Care is coordinated with cancer treatments."
