If you must take leave for your own health condition, including non-work related illness or injury, you may be eligible for disability benefits. This is in addition to the job protection that you receive through the Family and Medical Leave Act and California Family Rights Act.

If you need to use sick time for more than five consecutive working days due to injury or illness, you must apply for disability benefits.
1. Call Liberty Mutual at (800) 896-9375 or contact them via their website: www.mylibertyconnection.com (first time users, register using company code: stanford)
2. To avoid any delays, download and sign the Disability and Leave Authorization Card and give it to your physician. Or complete a medical authorization form at your doctor’s office or medical facility that allows Liberty Mutual to receive all relevant information about your medical condition.
3. Liberty Mutual will need your physician to provide certain information about your health condition in order for you to start receiving disability income
