If your spouse/domestic partner or child(ren) gain or lose their non-Stanford coverage, you may be able to update your benefits.

*Remember, your medical plan eligibility is based on your home zip code. You must make changes within 31 days of the first day your new address takes effect, and if necessary, select a new medical plan.

1. Submit document(s) to verify dependent eligibility.
Within 31 days of your life event submit your documentation via fax to 855-818-3246.
If documentation is not received your dependents coverage will be canceled retroactive to the date of the life event.
