Spanish Chronic Disease Research Instruments
These instruments were used for the Spanish Chronic Disease Self-Management Study, Tomando Control de su Salud.
You may use any of these scales at no cost without permission.
We give you the items, their properties (if available), and coding and scoring instructions. They area available both on web pages that you can print directly (using the "Printer-friendly" link at the extreme bottom of each page) and as downloadable PDF files. Each page has a download link at the bottom to save the PDF document to your hard drive. The Downloads web page includes links to each PDF scale, as well as to sample questionnnaires.
