H4A Seminar: Integrated Care in the Field of Aging

Date & Time: Saturday, August 13, 2016: 9am-1:15pm (with breaks for coffee/snacks)

Schedule: Half-day seminar runs from 9am-1:15pm and includes a morning break from 11-11:15am. The seminar presenter may need to change the break times, if needed.  

Location: Li Ka Shing Center, room 120, Stanford School of Medicine, 291 Campus Drive, Stanford, CA 94305. Note: the Li Ka Shing Center is identified as the Stanford Medical School on the hyperlinked map.

H4A seminar agenda: With the promise of increasing longevity, there is a great need for integrated care, planning, research, and education in the field of aging. This seminar will explore how prevention, current care models, advance care planning, disease-specific interventions, and community-based programs can enhance wellness for aging adults and adult children caring for their elders. This seminar will cover these specific topics:

  • Physical and psychological correlates of aging
  • Current care models (including a discussion of case studies)
  • Advance care planning
  • Chronic disease self-management through the life course
  • Dementia management through the life course
  • Technology solutions for aging in place
  • Understanding the transposition of roles in families as elders age.

 

What will you learn: This seminar will share information on:

  • The critical gaps in care for aging adults and adult children caring for their elders
  • The key health care principles at play through the continuum of care
  • Developing effective partnerships with community and academic organizations (as well as organizations from other sectors) to best serve aging adults and those that care for them
  • Implementing key strategies for effective care models
  • Developing collaborative employee health programs for those struggling with eldercare
  • Designing innovative products for aging in place
  • Effective models of knowledge dissemination and implementation for universities, hospitals, and communities.

 

Who should attend: This seminar will be useful to anyone who cares for or provides for an aging adult(s), is an aging adult, or is working in/has an interest in the field of aging and the continuum of care through the life course. Seminar content will be accessible and relevant to:  

  • Interested community members
  • People working at non-profit community centers, medical centers, and government agencies
  • Students (including medical students)
  • Researchers and scholars working the field of aging or related fields.

 

Cost: Half-Day Seminar Rates:

  • Stanford STAP eligible.
  • Early Bird Rate = $250 USD (through July 30)
  • Scholarship Rate = $250 USD (includes students, trainees, H4A alumni, Stanford employees (STAP eligible), SHPN members, URMs, government agencies, and non-profit community partners)
  • Regular Rate = $375 USD
