Funding Priorities
The City of San José Housing Department administers the Housing Trust Fund ( HTF) program .The program is available to nonprofit agencies that provide services to people who are homeless or at risk of becoming homeless. The funding priorities for the HTF program include:
- Destination: Home sponsored projects
- One-time only office equipment purchase, facility emergency repair, or gap funding for shelters or other homeless service providers
- Emergency fund for persons displaced due to fires or other such disasters
- Programs geared toward preventing or ending homelessness including those that provide financial assistance, employment services, and case management
- Programs that will permanently house chronic homeless households with case management and other supportive services

Downtown Homeless Outreach & Engagement Project |
$347,000 |
Project provides outreach and engagement services to homeless individuals living in the City of San José’s downtown area. Through targeted outreach and engagement services, the Project assists homeless individuals to transition from life on the streets into more stable housing. In order to achieve this goal, the Project provides street-based outreach and engagement, emergency shelter, intensive case management, financial assistance, and vocational assistance. This project was funded through December 2015. |

Destination Home Case Management (Housing 1000) |

As one strategy of Santa Clara County’s Community Plan to End Homelessness, Destination: Home will create a capital campaign to raise funds to develop approximately 6,000 housing opportunities for homeless men and women. Destination: Home is a program of The Health Trust. |

Care Coordination Project and HMIS |
$1,040,000 |
This project provides funding for related activities of the Care Coordination Project (CCP), including the continued support of supportive services for approximately 160 chronically homeless households and ongoing operations of the Help Management Information System (HMIS). |

Food Study |
$50,000 |
This project completes a Food Access Plan that augments existing data with primary data collection and that will serve as a guiding document for the City of San Jose and other food access stakeholders to understand how best to address food access needs in San Jose. |
