Communication with Referring Physician

Most patients are referred by their physician for consultation. We take care to provide referring physicians with a comprehensive summary following the first visit and status reports periodically thereafter. Return appointments can be made at the time of checkout after a clinic visit. Our clinic staff will obtain authorization for both new and return appointments when such authorizations are required. Please assist them by requesting a referral from your primary physician if one is required.

First Appointment and Registration

Please arrive at clinic one hour before your initial appointment to facilitate registration and have your pre-appointment lab work drawn. Most patients have a complete blood count drawn prior to clinic so that blood slides are available for review at the time of your visit. Our check-in staff will provide you with the appropriate lab slips.

The Hematology Clinic and blood drawing lab are on the first floor of the Stanford Cancer Institute.

Please allow 2 to 3 hours for your initial evaluation, including your registration and blood work. You will be seen by a faculty physician and may also be seen by another physician or medical student on the Hematology Service. Additional tests may be requested after your visit. In many cases these can be obtained by your referring physician if this is more convenient for you.

What to Bring To Your Appointment

We ask that you hand carry the following items to your initial visit unless your physician has sent them prior to your visit,

• All pertinent medical records including a referral letter if available. Your referring physician's office can usually provide assistance in obtaining this information.
• Bone marrow slides and pathology reports (if applicable).
• All pertinent x-rays and CT scans.
• Health insurance information. We will try to obtain this in advance of your visit.
• Name, address and phone number of your referring physician.
