Health and Safety
The Medical School Human Resources Group is a resource for some Health and Safety issues. Consult with Employee Relations staff when a health and safety issue involves an employee who:
- has sustained a work-related injury,
- is unable to work, or
- has work restrictions or requests for work accommodations.
Contact Risk Management for questions regarding reporting an injury.
As a reminder, when an employee injury occurs, complete the following forms:
- Workers' Compensation Form, DWC-1 (available from Risk Management Department, 723-7400)
- Incident Report, SU-17
- Employer First Report of Injury, Form 5020
- Report of Lost Time, SU-16
