Assembly of Professoriate long forms starting April 15, 2017
To get started:
- Click the "Launch ByCommittee" button
- On the home page, click "Sign In"
- At the bottom of the drop-down window, select "Or sign in with Partner Institution"
- Select "Stanford University" from the menu
- Log in via WebAuth as you would to any secure Stanford site.

Stanford University's Faculty Academic Staff Appointments web form system must be used for:
The professoriate and emeritus faculty for these types of transactions:
- Add a department, school, or policy center/institute affiliation
- Extend an appointment
- Change the percent time of appointment
- Provide notice of a resignation, retirement, termination, death
- Recall Emeriti (FRIP and non-FRIP)
The clinican educator and all other teaching title faculty for all transactions except:
- Changes in duties
- Salary changes

FASI
(Faculty Applicant Self Identification)
Used during faculty searches, FASI gathers data from your candidates about their gender and ethnicity, and generates the Applicant Information Grid for inclusion in the Search Report. The University Provost's office administers the FASI system.
