The University long form is a compilation of highly confidential materials used in the review of a faculty member for appointment, reappointment, or promotion.  ByCommittee is the system used to assemble the long form. The School of Medicine has a set of requirements for the composition of the referee grid, evaluation of clinical skills, and other pieces of evidence assembled in the long form.

Search Initiation Requests

• Search Initiation Request Form - fill out this form online and submit it via email (with additional information as needed) to the Office of Academic Affairs to request permission to initiate a Professoriate search.

Financial Forms

• Form I-9 - Federal Employment Eligibility Verification (INS Form I-9):
  The USCIS has also released a new Employer Handbook  for the I-9 process.  The Handbook contains instructions for completing the I-9 Form, sample images of the acceptable documents and FAQs.

Teaching Evaluation - Template Surveys
