- When to Schedule an Appointment

Schedule your appointment at least four to six weeks before departure to allow enough time to receive the recommended vaccinations. Travelers with identified risk factors and/or disease symptoms should make an appointment in Medical Services for a pre-travel assessment.
- Preparing for Your Appointment
- Bring your immunization record and/or your International Certificate of Vaccination (the “yellow card”) with you. Having a complete and current immunization record can help reduce the expense of receiving unnecessary immunizations.

Complete our travel health history questionnaire on VadenPatient prior to your appointment, or print out and complete the form (International Travel Health History form – PDF format) and bring it with you to the appointment.
- Cancellation and No-Show Policy
- Up to one hour is reserved for your appointment. There is a no-show charge for patients who cancel an appointment with less than 24 hours notice or who do not show up for their scheduled appointment.
